II. REQUIREMENT FOR WORKING VISA APPLICATION IN SHENZHNE

1.Original passport(info pages including front paper, last Chinese visa, last entry paper; the validity of passport should be within 6 months)

2.Application Form for work permit(origin/copy;with signature and official seal)

3.Physical examination report(Chinese origin or foreign scan; issued by China Inspection and Quarantine Institution or overseas health and medical organization approved by it, with the issuance date within 6 months)

4.Notarized and endorsed highest academic degree certificate/Notarized and endorsed professional qualification certificate (if any).

5.Working experience certificate.(issued by the former company with signature or seal and the valid contact info)

6.Notarized and endorsed non-criminal records certificate

7.Labor contract or offer of employment（including working time,location,job position,description and salary, with company seal and signature）

8. Six recently taken two-inch white background certificate photos and receipt

9. Business license copy of the employer company with seal

10.SZ employer company connecting person ID copy, telephone, fixed telephone and legal representative ID
